Telemedicine — New Patient

New to the practice and want to start online? We offer full new-patient intakes over our secure video platform. Your provider will review your history in detail, establish a care plan, and — when needed — arrange labs or an in-person follow-up.

Good to know:

• We'll email an intake form ahead of time
• Bring a current medication list and any recent lab results
• If your visit requires a physical exam, we'll schedule an in-person follow-up
